During the checkout process there is an "Installation" Option for an additional $49 to have someone install the template for you on your server. Usually we have a third party do that. I would recommend this way as it can save you a bit of money. You will find that during the checkout process there are a few other options as well that may be a bit cheaper that you may be interested in.
If you want Ozzu to do it for you, we charge $150/hr for custom work. So the total amount would depend on exactly how much you wanted us to change for you. If you simply want us to setup the template on your hosting account to be the same as on the demo as shown above, that would take about 30 minutes and would cost $75 additional.
