Cover the words

I am trying to create the effect where the word is shielded, and making it unclear. I don't know what do we call this, Can anyone help with this?

What do i use to create this effect?

It's hard to see from this attachment, but do you mean to completely blur the text?

Blur it completely.

I used to use the black ink and sketch over it, it look really messy and childish. So now, i'm thinking to use this way which i have seen many usually will do.

i think there must be some sort of tool in photoshop or something...?

Try a fiew of the blur filters
For example this was done with Filter>Blur>Box Blur set at 10 pixels

A vertical motion blur would probably give you more consistent results with the one above.
