This is the quick and dirty way to fix it (without going into Bios)
right click on MY COMPUTER,select properties, Hardware ,Device
Manager , Double click on Floppy Disk Drives, then when the same,
appears underneth that one ,Right click on it ,and select UNINSTAL,
the the OKAY button, and reboot, when it comes back up ,it will
find new hardware ,,,and just hit next ,next ,and it should re-install ,
back ,and end it's problem .

This is almost always because the ribbon cable from the motherboard to the floppy drive is on backwards, other then that the drive is bad.

I have NEVER seen this from a software or OS issue. It's always a hardware problem.
