Apache, Webmin, php: service needed

Does somebody provide these services or know someone who does?
1. To redirect 301 ten own domains
2. To correct a php form on two sites that is giving 404 error

Redirect 301 - I did many trials and am unable to get them as want to

I need redirect permanently:

From domain.com to http://www.domain.com/
From http://www.domain.com to http://www.domain.com/
From domain.com/index.htm to http://www.domain.com/
From http://www.domain.com/index.htm to http://www.domain.com/
From http://www.domain.com/folder/index.htm to http://www.domain.com/ folder/
From http://www.domain.com/folder/ folder/index.htm to http://www.domain.com/ folder/ folder/

Pages with extension index.htm are common because the sites were constructed on Dreamweaver.

The sites are hosted in a Unix Virtual Server – FreeBSD with a unique IP address, Apache 1.3.33, with Webmin installed.

I`d like, if possible, to put general directives instead instruction for the sites separated. If it`s impossible, ok for .htaccess. I have already installed .htaccess files for Urchin.

Please point a similar service already done.

The php form is simple and are used to order by email (not banking or card transaction) two books.
