Web Hosting recommendations

Hello. Been a long time since I have posted here. Main reason I had no web site 
Anyhow, things are getting back on track now for me and I would really appreciate some recommendations for a good web host. I know I will get good feedback from ozzu, so this is the only forum I have posted asking this.
What I need is something that will stay up with a very rare chance of downtime. Some reasonable bandwidth limits, I am not expecting a huge amount of traffic, but would like the option to purchase more at a reasonable price. I need about 1000mb of space, PHP and MySQL. I would like to spend about ~15$ a month for a plan.
