php coding problem - reference numbers on forms

The form is commented out. Therefore I cannot enter values into it......

I just turned error reporting up to full:

Which is fine because that is $_POST['submit'] - this is not an error as such, just a notice, nothing to worry about.

actually i ran it again and i get a QUERY WAS EMPTY". is that the validation if all fields havent been filled in?

it doesnt list the errors. in the code it was to ask 'where do you live?' or 'whats ya name, retard' but it doesnt say any of those things. instead it says QUERY WAS EMPTY. thing is the query wasnt empty at all.
