comments with rating

I need to put up a comments / guest book page where clients can post their experiences with the business.

I usually just use advanced guestbook for a client comments page but I need the abiltiy to add something like a 1 to 5 star rating with the text comments.

I don't know how to create that so anyone know where I could get a script for something like that?

Yeah went there first, they have a comments & rating section but didn't see one I liked.
