Do you know of a good CSS/JavaScript tutorial covering methods for duplicating the functionality of sample form below? I know how to do everything with the exception of dynamically adding an unlimited number of new rows which may contain grouped OR clauses.

Um... not really. I just type everything out... what exactly are you looking for? After all, a GUI tool isn't doing anything you can't do with your own 10 (presumably) fingers. The result is still just a text file...

like i described in another one http://dynamicdrive.com but that might not be the best becasue they just tell you what to do and might not have what you want..... i use it but im trying to learn without it but it helps. i look at the code for a while till i think i understand then put it on my site. if i dont undersatnd i ti usually dont put it on.

I don't want to invest too much time learning yet another technology. Work is already getting complicated juggling: PHP, Perl, VB Script, HTML, CSS, a dozen heavy programs - all for development on NetWare, Linux, and Windows. DHTML will have to wait. :-P

This is a one time problem; I'm hoping I can find a sample/tutorial that can be tweaked to suit my purposes.

The form must change as the user makes selections. Take another look at the top image. Notice how the and/or/"end group" selections add/delete rows. The DHTML form I need must be able to re-produce this effect.
