Can you recommend some simple application for collecting feedback from website visitors? I would like to put it in my website and get visitors opinions on how my website works. I couldn`t find anything interesting yet.

If you collect email address for newsletter then you can send them a survey or put the survey on the website and offer some type of prize(s) to one lucky visitor who completes the survey.

Maybe a PHP form with fields for whatever info you want to collect? You could have the results go to your email.
