front page form problems

I am having some trouble w/ a frontpage form at http://www.rfjusa.com/feedback.htm
Cannot run the FrontPage Server Extensions on this page: "http://www.rfjusa.com/Feedback.htm"

can anyone help me?

Make sure your server can run FrontPage extensions, and don't just copy the html to the site, but publish the site via FrontPage.

The server is running the extensions and I have published via frontpage. that didn't help me any. Any other ideas?

I also have dream weaver but being new to it I don't understand how to work it. could I just open the file created in frontpage and change something (I am not sure what though)? And then upload that either w/ frontpage or FTP? All help is greatly appreciated.
